Stroller Order Form

By signing below:

I hereby authorize Family Care Options by filling out and signing this credit card authorization form to charge the credit card provided for any and all services rendered and/or ordered by Family Care Options, Inc. The credit card provided will be charged in such a manner as if I were paying in person and signing a transaction slip. I understand there will be a 4% service fee for all phone and internet credit card orders.

Cancellation Policy:

Cancellations received less than 48 hours before the scheduled start date, the client will be responsible and billed for the entire schedule of services confirmed. All daily rates will be charged to the credit card provided. Cancellations received more than 48 hours in advance will receive a refund.

Method of Payment:

All major credit cards accepted and due upon confirming the care.

Reservations:

The reservation of the stroller is based on availability, therefore, we do not guarantee the dimensions, size, brand, manufacturer or color of the stroller requested.

Stroller Return: I understand and agree that at the end of the stroller rental reservation, I will return the stroller to a representative at the concierge desk.

Please note: In the event the stroller is not returned to a representative of the concierge team at the end of the stroller rental reservation, you will be responsible for the replacement of the stroller and the credit card on file will be billed for a replacement stroller fee of $550.00.
